Participation Information & Checklist for the CARE Summer Bridge Program

Congratulations on your acceptance to Florida State University and the Center for Academic Retention and Enhancement (CARE). As a CARE student, you will attend Florida State starting in the summer of 2022 through the Summer Bridge Program, a seven-week program. During this time you will live in on-campus housing with CARE staff and ambassadors, and engage in a variety of activities designed to support your success as an FSU Seminole! We look forward to offering you an excellent learning environment and a memorable beginning to your college experience.

After the summer term, you will remain in CARE and Florida State, participating in a variety of activities and programs designed to promote your academic engagement and progress through to graduation from the University.

By accepting your admission to FSU through CARE, you have the benefits of

• Tuition, Fees, Housing, Meals, Books, and Insurance Covered for Summer 2022!
• No Admissions Deposit!
• No Orientation Fee!
• No Summer Housing Deposit!
• Only a $50 Fall/Spring 2022-2023 Housing Contract Prepayment

You will receive several communications over the next few months leading up to the start of Summer Bridge which will address academic advising, move-in processes, financial aid, and additional resources. With Summer Bridge, you must complete the entire seven week program, June 12 to July 30 with move-in of your choice on June 10 or 11.

Next Steps. You should read everything in each step to be sure you don't miss any valuable information!

Failure to complete the items below by the due dates will result in your offer to participate in the 2022 CARE Summer Bridge Program being rescinded and your admissions to be canceled!

Checklist for CARE Admissions

1. Complete the Participation Agreement (Due by May 1, 2022).

• You can turn this in at any point in the admissions process.
• Final Due Date: May 1, 2022.
• The Participation Link is on your Status Check page or the Participation Agreement.
• Failure to return the agreement by May 1 will result in automatic cancellation of your participation in CARE and the Summer Bridge Program.
• Although the agreement is due on May 1, the other items must be received by their due dates in order to ensure your offer remains valid.

2: Activate your FSUID (must be done to complete all other items)

• Your FSUID will be your username and email address for FSU. This is also how you log into the FSU system to review your To-Do List, Complete Housing Documents, See your Verification Forms, etc.
• To activate your FSUID, go to https://fsuid.fsu.edu/OIMSelfService/.
• Most items cannot be completed without this step completed.
• You will need your EMPLID and your Social Security Number to activate your FSUID.

3: Complete the CARE First Generation Verification Form (Due March 14, 2022)

• As a CARE student entering through the Summer Bridge Program, you must be a First-Generation College Student.
• To verify your eligibility, you must complete our First Generation Verification Form.
• The form will be emailed to you at your email address on or about March 8.

4: Complete Registration for FS4U Foundation Scholarship System (CARE Deadline: April 5, 2022)

• Go to https://fsu.academicworks.com/ to complete your registration for the FS4U Scholarship System.
• The FSU Foundation's scholarship management system is a way for CARE students to search for and find many scholarships that FSU has to offer. CARE students will also be able to connect to scholarships that are specifically awarded to our students.

Checklist for Financial Aid

1. Complete 2021-2022 and 2022-2023 Free Application For Financial Aid (FAFSA) (Due March 1, 2022).

• BOTH a 2021-2022 AND a 2022-2023 FAFSA are necessary for the Office of Financial Aid to develop your financial aid award.
• The 2021-2022 FAFSA is used for your Summer 2022 financial aid award, and the 2022-2023 FAFSA is used to create your Fall 2022, Spring 2023 and Summer 2023 awards.

2. Provide any Financial Aid related documentation requested (Due April 6, 2022).

• Financial Aid may need additional information from you so please make sure you check your official FSU email and the email you provided on your admissions application.
• On or about March 14th, you will be notified if you have been selected for Verification, which is a process used to confirm information provided via FAFSA. All students are not selected for verification; It is a random process resulting in approximately 30% of students being selected. Continuously check your email and your "To-Do List" found at my.fsu.edu to identify any documentation that you need to submit to Financial Aid. If documentation is requested, you can click on each documentation request for submission instructions. documentation that The Office of Financial Aid will utilize to verify your eligibility for CARE.

  2. Additional information about submitting requested documents can be found here.
  3. If you do not have forms listed, then you are not required to do anything at this time. However, you should periodically check your to-do list.
• Make sure you are checking your official Florida State student email. The Office of Financial Aid will regularly send key financial aid information that is important to your transition to Florida State.
• Please note:
  1. All requested documents for 2021-2022 and 2022-2023 FAFSA must be turned in by March 28, 2022! This MUST be done even if you have not returned your Participation Agreement!
  2. Late submission of documents may cause your CARE offer to be rescinded.

Checklist for Housing

1. Complete Fall 2022 /Spring 2023 Housing

• Complete the FSU Fall Housing Undergraduate Application (and $50 Student Prepayment) -ONLY IF YOU PLAN TO LIVE ON CAMPUS FOR THE FALL 2021-SPRING 2022 YEAR. This is an optional step.
• To contract for housing, complete the following steps in order:
  1. Complete the housing contract beginning on February 23, 2022.
  2. Within 7 days, an advance rent payment of $50 is due.
  3. Also within 7 days, applicants under 18 years of age at the time of contract submission must complete and return the parent/guardian signature form which is available on the Housing website.
• Students who contract on February 23rd or February 24th will be entered into a lottery for their contract number and receive their contract number on February 25th. Students completing a contract on February 26th and after will immediately receive a contract number. The contract number serves as your "place in line" for receiving on-campus housing! Housing is very limited, so apply early if you wish to live on-campus for the Fall/Spring terms.
• We encourage all first time freshmen to live on campus for reasons of retention and academic performance.
• Please note: Housing is unable to refund the difference between the regular and CARE housing prepayments whether the larger prepayment was made in error or prior to acceptance in the CARE program. If you do make the $225 prepayment, you will have a smaller rent balance after you are assigned to a room. This means that either you will owe less or you will receive more funds when your financial aid pays out, so in the end you still pay the same amount for your rent.
• A few important points for you:
  • On-campus housing is not required or guaranteed for incoming CARE students at FSU (although it is strongly preferred).
  • The housing contract is binding if you attend FSU.
  • The housing contract does not commit you to FSU. If you are considering coming to FSU and think about staying on-campus, you can complete the contract to reserve your spot just in case.
  • Failure to complete Housing Steps pay $50 advanced payment or submit parent/guardian signature page within the specified time will result in cancellation of your Housing contract. You would then need to resubmit the contract, resulting in a change to your contract number.
  • You can submit a preferred roommate request between March 7th and April 13th. You will select your Fall/Spring room online starting in mid-May via the FSU myHousing portal.

  2. Complete the MANDATORY Summer Housing Application (Opens MARCH 2, 2022; CARE Deadline: APRIL 14, 2022).

  • The Summer B contract will open March 2, 2022. The CARE deadline is April 14, 2022.
  • Complete the FSU Summer Housing Contract. THIS IS MANDATORY.
    • ALL CARE Summer Bridge Program Participants are required to reside in the assigned residence hall for the entire duration of the summer B semester.
    • You are not required to pay the $100 prepayment! If you pay the $100 fee, Housing will NOT refund you the money.
    • As a CARE student, you will live in Magnolia Hall for the Summer B Term.
    • Please note that the Summer and Fall/Spring contracts are separate. As a summer admit, you will need to complete BOTH if you plan on living on campus for the Fall/Spring term.
    • Fall on-campus housing is NOT required or guaranteed for incoming students at FSU. If you want to stay on-campus for the Fall, you should sign-up EARLY.
    • Students must reside in the designated CARE residence hall for the duration of the Summer Bridge Program. Students WILL NOT be allowed to live off-campus or in an on-campus residence not assigned by CARE or University Housing.
    • The housing contract is binding only if you attend FSU. The housing contract does not constitute a commitment to attend FSU. You still must complete the Participation Agreement.
    • CARE students WILL be able to select one roommate for the Summer Bridge Program. You can submit a preferred roommate request between March 2nd and May 1st.

    3. Off-Campus Housing Fall 2022/ Spring 2023.

    • As previously stated, living on-campus provides great benefits for your success at Florida State University. You will be close to campus resources and events which will allow you to be a more engaged students. However, if you decide to live off-campus, you are encouraged to make smart decisions.
    • You're encouraged to not sign an off-campus lease until you know your Fall 2022/ Spring 2023 financial aid award to prevent you from signing a lease over your budget.

    Checklist for Health Compliance

    1. Submit Your Proof of Insurance (CARE Deadline: June 1, 2022).

    • FSU requires all students to have health insurance. You must show proof that you have health insurance to take classes here. If you have the health insurance hold, you have 3 options:
    • Upload proof of your health insurance into the system at https://studentinsurance.fsu.edu/insurance-requirement if you have insurance for Summer 2022.
      • Click on “I Already Have Health Insurance” and follow the steps.
      • To purchase the insurance through the University for Summer 2022, go to https://studentinsurance.fsu.edu/insurance-requirement.
      • Click on “I Am Interested in Purchasing Health Insurance” and follow the steps.
      • Because you are in CARE, if you purchase the summer health insurance, the CARE financial aid package will cover your summer insurance costs.

      2. Submit the FSU Health Compliance Forms (CARE Deadline: June 1, 2022).

      • Go to https://studentinsurance.fsu.edu/immunization-requirements to review immunization requirements.
      • You will need to submit the Student Immunization Record.
        • You can send the form into FSU by one of these 5-ways:
          1. Email: healthcompliance@fsu.edu (Please be aware that email sent over the Internet is not considered secure. FSU shall not be liable for any breach of confidentiality resulting from this form of communication.)
          2. FSU Drop-Box: Go to http://dropbox.fsu.edu/ and upload the form to healthcompliance@fsu.edu
          3. Fax: Fax it to 850-644-8958
          4. Mail: Mail the forms to University Health Services, c/o Health Compliance Office, 960 Learning Way, Tallahassee, FL 32306-4178.
          5. In Person: You may also drop off your forms In Person to the Health Compliance office at UHS during regular business hours https://uhs.fsu.edu/about/contact-us at 960 Learning Way

        Orientation Programs

        • Orientation Dates: CARE students all attend the same University Orientation session. Your Orientation will take place Sunday, June 12 – Tuesday, June 14, 2022.
        • Starting at State (Pre-Orientation Modules): You must complete the online pre-orientation modules by the deadline provided by New Student and Family Programs.
        • Registering for Orientation: As a CARE student, you must complete the official University Orientation on June 12-14. You will be notified once registration is available for New Nole Orientation. You must complete registration for yourself. CARE will not register you.
        • Fees for Orientation: The Orientation fee is built into your costs to attend CARE. Although a bill will appear on your account statement, Orientation will be paid for when we get closer to the Move-In Day. There’s nothing for you to do, except complete the Participation Checklist.
        • CARE Family Orientation: There is a CARE Family and Supporters Orientation that will occur in-person on June 11 or virtually on June 12. For your family member or supporter to gain access we ask that you include them in your New Nole Orientation registration. The session is free and open to any family member or individual supporting you during your college transition and experience. This is an opportunity for parents, supporters and other family members to gain valuable information about Florida State and your college experience.
